Nucletron introduces a volume based optimization routine IPSA, which was already used for many years in its SPOT Pro system for permanent prostate planning and treatment, into its world-leading HDR and PDR Brachy planning system PLATO.


IPSA (Inverse Planning Simulated Annealing) is an anatomy-based inverse planning optimization algorithm developed by Jean Pouliot, Ph.D., Etienne Lessard, Ph.D. and I-Chow Hsu, M.D. at the University of California, San Francisco. IPSA is designed for 3D image-guided brachytherapy treatment planning and is now available for High Dose Rate brachytherapy as well.


The optimization process is guided by organ-specific dose objectives. These dose objectives are normally set to maximize the target dose coverage, while taking into account other clinical objectives such as the dose homogeneity and the protection of organs at risk. In order to solve this issue of optimization for clinical applications as efficiently as possible, a unique simulated annealing engine was designed.


IPSA allows the creation of class solutions that can be used as a starting point for every patient, thereby further reducing the time needed to plan for individual patient treatments. The concept of class solution is commonly used in IMRT.


IPSA has introduced this concept in High Dose Rate Brachytherapy. “IPSA is used for all anatomical sites typically treated with HDR brachytherapy. The general consensus is that IPSA generates plans with better dose coverage, enhanced protection to organs at risk and in a shorter time than with previous approaches,” said Jean Pouliot, PhD at University of San Francisco.


Dr. I-chow Hsu, MD at University of San Francisco quoted: “IPSA has given us improved control over dosimetry of our brachytherapy treatment. It opens the possibility to create new sophisticated treatments never considered before.”


PLATO Remote Afterloading software is Nucletron’s world leading Brachy software for High Dose Rate and Pulsed Dose Rate Brachy treatments delivered by the microSelectron HDR and PDR.
